Backport for r515914 and r516323 - Fix Stage Manager crash when using the Duplicate action in the RMB menu on a prim in the python panel. Fixed a bug where copying multiple items in the tree sometimes caused a null parent item due to it being weakref object. Fixed a bug where the tree was unnecessarily refreshed after a paste op, resrting the scroll position and selections. Centralized the visibility and activation icons in the tree. Fixed a bug where selecting items in the tree were being ignored due to recursive callbacks. Disabled double clicking on the visibility and activation columns.
