Hmm … I'd try a fresh new install of Houdini. Say, like one the daily builds.

This appears to be related to the help browser. Try unchecking the “Edit -> Open Help Browser at Startup” menu item.

The missing icons and the error message appear to be separate issues. Unchecking “Edit -> Open Help Browser at Startup” should avoid the the error message about SQLite. This is a step in the right direction but not a solution to all of the issues you described.
