File -> Install Operator Type Library
Click the + button beside the field to file browse and select the otl file you wish to install.
To use, press TAB in the network/viewer pane of the optype that the OTL is (object, SOP, POP, COP, etc) and select it from the list (typing the name is often the easiest way to find it).

If you only want it for a particular session, you can load it on the commandline too:
%> houdini some_otl.otl some_scene.hip
%> houdini some_otl.otl some_scene.hip
